CRMS Recognized for Quality Care Service

 Calais Regional Medical Services (CRMS) Chronic Care Management Service was selected out of eighty-eight submissions to be presented at the Centers for Medicare Services National Quality Conference this month. This CMS National Quality Conference is a premier learning event for nearly 2,500 attendees from across the healthcare spectrum. It is a great honor for the work of Dr. Peter Wilkinson and Sara Probert, CRMS MA to be presented and recognized for the quality outcomes and patient and family engagement the program attained. 

In March 2017, the staff at Calais implemented a chronic care management (CCM) test of change to reduce the incidence of repeated hospitalizations and emergency department utilization. The results were patients felt more respected and clinicians reported that patients adhered to discharge instructions. Additionally, shared decision making between care managers and patients improved. 

The CCM service is part of CRMS' work with the Northern New England Practice Transformation Network (NNE-PTN). CRMS joined the Network in 2016 and for the next two years will continue to work together to expand on its successes. The NNE-PTN is a tri-state program managed by Maine Quality Counts. The program supports practices and clinicians in Maine, New Hampshire, and Vermont in their efforts to transform the delivery of health care from a volume-based to a value-based model by providing technical assistance, peer-to-peer learning opportunities, education, and coaching.

The NNE-PTN is funded by a grant from CMS, which is one of the largest, multi-year efforts to transform the health care system to improve quality of care, increase patients’ access to information, and spend dollars more wisely.
